Kohl’s Healthy Rewards is an easy and free way to save $500 (or $1,000 if your spouse/same-sex domestic partner participates) on your 2015 medical plan contributions and learn more about your personal health. Check out these FAQs to find out what you need to know and do.

General and Eligibility

A. Healthy Rewards isn’t a program—it’s a way of life. Administered by Limeade, a trusted wellness partner, it’s designed to help you and your families improve or maintain your health through a variety of tools and resources. It’s free, optional and completely confidential. And, if you completed your online Well-Being Assessment and Health Screening by March 31, 2015, you can save up to $500 or $1,000 on your 2015 medical premiums.

For 2015, we’re changing the way we allocate your Healthy Rewards to put more focus on the opportunities for maintaining and improving your health.

A. Studies show that many of the causes of chronic conditions are lifestyle-related or could be managed or avoided altogether with routine preventive care. Kohl’s wants to help you proactively manage your health, so your life is better and more productive. And, a healthier workforce helps keep health care affordable for everyone.

A. There are two parts to Healthy Rewards—the medical premium credit and Healthy Rewards challenges and activities. Here are the eligibility rules for each part.

Medical Premium Credit

Only Associates and their spouses/same-sex domestic partners actively enrolled in Kohl’s medical coverage on or before March 1, 2015 will be eligible for the medical premium credit. To earn your 2015 medical premium credit, you needed to complete your online Well-Being Assessment and Health Screening by March 31, 2015. You also needed to meet 3 of 5 health range targets or complete the Your Health Matters coaching program (or, if you’re expecting, the Exclusively Yours maternity program). If you missed the March 31, 2015, deadline, you can still complete your online Well-Being Assessment and/or Health Screening during the year. Although you won’t be eligible for the 2015 medical premium credit, you’ll still benefit from valuable information about your current health status and earn points toward your Healthy Rewards points bank.

Healthy Rewards Challenges and Activities

All Associates (including Associates not enrolled in Kohl’s medical coverage and part-time Associates) and enrolled spouses/same-sex domestic partners who participate in Healthy Rewards can earn points for completing fun wellness activities.

A. If you completed your online Well-Being Assessment and Health Screening by March 31, 2015, you will begin receiving your medical premium credit in May 2015.

If your results show you meet 3 of 5 health range targets, you’ll receive a $500 medical premium credit beginning May 2015 and are encouraged to maintain and improve your results by participating in Healthy Rewards challenges.

If you don’t meet 3 of 5 health range targets, you’ll receive a $150 premium credit beginning May 2015—and can earn the remaining $350 premium credit beginning September 2015 by completing the Your Health Matters coaching program or, if you’re expecting, the Exclusively Yours maternity program by July 31, 2015.

Other parts of the Healthy Rewards continue throughout the calendar year.

A. No; HIPAA prevents Kohl’s from viewing any specific participant results. In fact, we’re working with Limeade, our trusted wellness partner, to administer Healthy Rewards activities. You completed your online Well-Being Assessment through the Healthy Rewards website. And, Limeade helped us make Health Screenings available at most large work sites. The only data Kohl’s will receive is who has earned a premium credit for payroll purposes.

A. No. Since your Health Screening is covered as preventive care, Kohl’s pays the full cost to offer Healthy Rewards to you and your enrolled spouse/same-sex domestic partner.

Online Well-Being Assessment

A. A Well-Being Assessment is a series of health and lifestyle-related questions you answer online, helping you learn more about your overall health and well-being. It gives you a snapshot of your health so you can both understand and manage your risk factors before they become serious issues.

A. Log on to yourtotalrewards.com/kohls and select Go to the Healthy Rewards Website in the top left-hand corner of the home page or go directly to the Healthy Rewards website at healthyrewards.limeade.com.

A. If you haven’t signed in before, select the Get started button and follow the prompts to set up your account. If you’ve signed in before but can’t remember your sign-in name and/or password, select Trouble Signing In?. Enter the email address that’s on file with Limeade when prompted and you’ll receive an email with a link that allows you to create a new password.

Need help logging on to the Healthy Rewards website? Refer to the 2015 Healthy Rewards Quick Start Guide that’s posted on the log on page of the Healthy Rewards website.

A. All you need is 15 minutes and online access to yourtotalrewards.com/kohls (log on and select Go to the Healthy Rewards Website in the top left-hand corner of the home page) or go directly to healthyrewards.limeade.com.

A. After you complete your online Well-Being Assessment you’ll receive a personalized report that outlines the risk factors or opportunities you can focus on to improve or maintain your health.

A. You needed to complete both the online Well-Being Assessment and the Health Screening by March 31, 2015 in order to start receiving the 2015 medical premium credit. You will not receive partial credit for only completing the online Well-Being Assessment.

A. You needed to complete your online Well-Being Assessment by March 31, 2015.

A. You will not receive the 2015 medical premium credit, but can still complete this helpful step. You can also earn points for prizes by completing fun wellness activities throughout the year.

Health Screening

A. A Health Screening is a simple test that measures key health factors like height and weight, blood pressure, cholesterol and blood sugar. It gives you a snapshot of your health so you can understand and manage your risk factors before they become serious issues.

This year’s Health Screening period ended March 31, 2015. If you didn’t participate, consider scheduling your annual physical with your doctor.

A. You will not receive the 2015 medical premium credit, but can still earn points for prizes by completing other fun wellness activities throughout the year.

Achieve or Work to Achieve Health Range Targets

A. See below for the factors being tested and why they are important:

Risk Factor Why It’s Important Healthy Rewards Target
Large waist Excess fat in the stomach area is a greater risk for heart disease than excess fat in other parts of the body. < 35 (female), 40 (male)
High blood pressure If your blood pressure rises and stays high over time, it can damage your heart and lead to plaque buildup. ≤ 130/85
Low high-density lipoprotein (HDL) HDL—sometimes called “good cholesterol”—helps remove cholesterol from your arteries. A low HDL level raises your risk of heart disease. ≥ 40 (female), 50 (male)
High triglycerides Triglycerides are a type of fat found in the blood. A high triglyceride level or being on medication to treat high levels is another risk factor. < 150
High blood blood sugar High or even mildly high fasting blood sugar may be an early sign of diabetes. ≤ 100 (fasting)

If your results show you meet 3 of 5 health range targets, you’ll receive a $500 medical premium credit beginning May 2015 and are encouraged to maintain and improve your results by participating in Healthy Rewards challenges.

Work Toward Achieving 3 of 5 Health Range Targets

If you don’t meet 3 of 5 health range targets, you’ll receive a $150 premium credit beginning May 2015—and can earn the remaining $350 premium credit beginning September 2015 by completing the Your Health Matters coaching program or, if you’re expecting, the Exclusively Yours maternity program by July 31, 2015.

The amount of your 2015 medical premium credit depends on how many eligible participants in your household completed the required activities and achieve or work to achieve their health range targets.

Premium Credit

You will begin receiving your medical premium credit beginng May 2015 through December 31, 2015 as long as you and/or your spouse or same-sex domestic partner are actively enrolled in Kohl’s medical coverage on or before March 1, 2015. Enrollment in COBRA does not apply.

A. It will take you six weeks to complete the Your Health Matters coaching program.

A. The Your Health Matters coaching program needs to be completed over six consecutive weeks. You can’t stop and start over a longer period of time.

A. Starting the six-week program by June 10, 2015 will allow you to complete the coaching program by July 31, 2015.

A. You will begin receiving your medical premium credit beginning May 2015 through December 31, 2015.

A. For 2015, your results will affect the way your rewards are allocated. The total amount that’s available is still the same, and all enrolled Associates and spouses/same-sex domestic partners have the opportunity to earn this amount regardless of their results. However, if you are identified as a candidate for coaching, you will need to complete that step to earn the full credit. If you or your enrolled spouse/same-sex domestic partner are unable to complete the coaching program, alternatives are available.

Here’s how earning rewards will work:

If you completed your online Well-Being Assessment and Health Screening by March 31, 2015, you will begin receiving your medical premium credit in May 2015.

Meet 3 of 5 Health Range Targets

If your results show you meet 3 of 5 health range targets, you’ll receive a $500 medical premium credit beginning May 2015 and are encouraged to maintain and improve your results by participating in Healthy Rewards challenges.

Work Toward Achieving 3 of 5 Health Range Targets

If you don’t meet 3 of 5 health range targets, you’ll receive a $150 premium credit beginning May 2015—and can earn the remaining $350 premium credit beginning September 2015 by completing the Your Health Matters coaching program or, if you’re expecting, the Exclusively Yours maternity program by July 31, 2015.

A. You’ll still be eligible to earn up to $500 of the medical premium credit. If you both partitcipate, you can save even more—up to $1,000 ($500 each)!

A. You will be awarded $650 beginning May 2015:

  • Associate completed their online Well-Being Assessment and Health Screening by March 31, 2015 and meets 3 of 5 health range targets = $500
  • Spouse/same-sex domestic completed their online Well-Being Assessment and Health Screening by March 31, 2015, but does not meet 3 of 5 health range targets = $150

You will be awarded a $350 premium credit beginning September 2015 if your spouse/same-sex domestic partner completes the Your Health Matters coaching program or, the Exclusively Yours maternity program by July 31, 2015.

Engage in Healthy Rewards Challenges Throughout the Year

All Associates (including Associates not enrolled in Kohl’s medical coverage and part-time Associates) can participate in Healthy Rewards and earn points for joining and completing fun wellness activities. The points you earn are added together (or banked)—and depending on the total, Kohl’s will offer different prize drawings at each level.

Get started: The Healthy Rewards points bank and many of the activities and Healthy Rewards challenges are managed by Limeade. You can visit the Healthy Rewards website to:

  • Complete your online Well-Being Assessment (All Associates and enrolled spouses/same-sex domestic partners) and Health Screening (Associates and spouses/same-sex domestic partners enrolled in Kohl’s medical coverage only).
  • Participate in Make Your Move quarterly challenges and activities—and even engage with other Associates who are also participating.
  • Set and track your individual health goals.
  • Follow your annual points total and progress toward each level.

A. Each activity is assigned a points value and as you complete the activities, you accumulate more points. There are plenty of activities and Healthy Rewards challenges available—everything from visiting the dentist to tracking your daily exercise.

Your points total takes you to one of three levels—each with its own choice of prize drawings. Choose the activities that appeal to you—and watch your points grow!

  • Level 1: First Steps = 1,000 Points

    Associates enrolled in Kohl’s medical coverage automatically receive 1,000 points if they completed their online Well-Being Assessment and Health Screening. All other Associates can complete their online Well-Being Assessment and other fun wellness activities.

    Your Rewards*: If you achieve Level 1, you have the chance to win a Go Pro camera, NatureBox subscription, Beats in Ears headphones, portable blue tooth speaker or Spafinder gift card.

  • Level 2: Hit Your Stride = 2,000 Points

    Associates who complete additional activities and Healthy Rewards challenges to earn more points for a choice of prizes.

    Your Rewards*: If you achieve Level 2, you have the chance to win two wellness days off (Associates only), Vitamix, Microsoft Xbox One bundle, KitchenAid Pro Stand Mixer or Apple iPad Air (16 GB).

  • Level 3: Lead the Way = 3,000 Points

    Associates who complete enough activities and Healthy Rewards challenges to achieve the highest level of participation (Wellness Star status) become eligible for the grand prize drawing.

    Your Rewards*: If you achieve Level 3, you have the chance to win free Kohl’s medical coverage (Associates enrolled in Kohl’s medical coverage only), three wellness days off (Associates only), a new wardrobe/shopping spree ($1,500 value), Samsung 50” 3-D Smart HD TV, strength training home gym or Microsoft Surface Pro 3.

Associates can earn points toward their Healthy Rewards points bank until November 30, 2015. The annual drawing for prizes will take place in December 2015.

A. You are eligible to earn prizes quarterly by participating in Make Your Move designated challenges. The points you earn for completing healthy activities and Healthy Rewards challenges will go toward your annual points and annual drawing for prizes. And now you will also be eligible for quarterly prize drawings when you participate in Make Your Move challenges. You need to earn at least 150 points to qualify.
