Complete Your Health Screening

A Health Screening is a free and easy way to determine your key health factors—including height and weight, blood pressure, cholesterol and glucose. When combined with your Well-Being Assessment, it gives you a full picture of your overall health.

You must complete your free and confidential Health Screening by March 31, 2015 to start earning your Healthy Rewards. Here’s what you need to know.

How to Complete Your Health Screening

There are a few easy ways to complete your Health Screening. Choose the option that’s most convenient for you—these options are free since this is preventive care.

For retail store Associates, complete your Health Screening at:

  • A CVS MinuteClinic (Visit minuteclinic.com and click the Store Locator link at the top of the home page to find your closest location or download the CVS mobile app and click the MinuteClinic icon to use the “Find a clinic” feature).
  • Another retail convenience clinic (including Healthcare Clinic, Target Clinic, FastCare and Walgreen’s Take Care Clinic).
  • Your doctor’s office. If you’ve visited your doctor within the last six months (July 2014 or later) and received health readings during your recent visit you can use these results.

For all other locations, complete your Health Screening at:

  • An on-site screening event at your work location
  • Your Kohl’s Wellness Center (if applicable)

Watch for more details about on-site screenings and screenings at the Kohl’s Wellness Center (if available in your area). You can get more information and register starting now. Screenings begin February 2 and run through March 31, 2015.

If these two options don’t work, you can print a Physician Fax Form from the Limeade website and take it to a CVS MinuteClinic or other retail convenience clinic or your regular doctor. (Visit minuteclinic.com and click the Store Locator link at the top of the home page to find your closest location or download the CVS mobile app and click the MinuteClinic icon to use the “Find a clinic” feature.)

Select locations, like the Corporate Office, Near Sites, New York and Santa Monica Design, West Coast IT, Credit, DCs and eFCs will have on-site Health Screenings available for Associates. Enrolled spouses/same-sex domestic partners cannot participate in on-site screenings due to security procedures at your work locations.

Remember, if you participate in an on-site Health Screening, you do not need to submit a Physician Fax Form.

To Participate: If online registration for on-site Health Screenings is available, you’ll receive an email with more information. If online registration is not available, you’ll receive information about paper sign-up at your work location.

Associates and spouses/same-sex domestic partners at select locations can access their Kohl’s Wellness Centers to complete their Health Screening.

Remember, if you visit a Kohl’s Wellness Center for your Health Screening, you do not need to submit a Physician Fax Form.

To participate: Call the Wellness Center to schedule an appointment (San Antonio, San Bernardino DC/eFC, Patterson or Dallas: 855-850-0246 or Corporate and Near Sites: 262-252-1050).

If you’ve visited your doctor within the last six months (July 2014 or later) and received blood pressure, cholesterol, glucose and height/weight readings during your recent visit you can use these results. If not, schedule an appointment with your doctor to complete these tests. Remember, your Kohl’s medical coverage will cover your screening results if taken as part of your annual preventive routine physical.

To Participate: Call your doctor to confirm you received the following tests within the last six months or schedule your annual preventive routine physical to complete these labs prior to March 31, 2015.

  • Blood pressure
  • Cholesterol (Total Cholesterol, HDL, LDL, Triglycerides)
  • Glucose
  • Height/weight

Note: Participants must fast 8-12 hours prior to their appointment for an accurate cholesterol and glucose reading.

When getting lab results from your doctor, your doctor must complete the Physician Fax Form. You can find the form on the Limeade website by following these steps:

  • Log on to yourtotalrewards.com/kohls and select Start Earning Your 2015 Healthy Rewards in the top left-hand corner of the home page or go directly to the Limeade website.
  • Select the Complete Your Health Screening tile on the Limeade home page to access the Physician Fax Form.

You’ll need to download the form online, print it and bring it to your appointment. Simply ask your doctor to fax in the form (as instructed on the cover sheet) to the number listed on the form by March 31, 2015.

Savers Plan, Basic or Traditional Plan participants can visit any MinuteClinic, the walk-in medical clinic inside select CVS pharmacy locations or other retail convenience clinics like a Walgreen’s Take Care Clinic. Make sure to look up the closest MinuteClinic at minuteclinic.com because not every CVS store has a MinuteClinic.

To Participate: When getting lab results from a retail convenience clinic, your health care professional must complete the Physician Fax Form. You can find the form on the Limeade website by following these steps:

  • Log on to yourtotalrewards.com/kohls and select Start Earning Your 2015 Healthy Rewards in the top left-hand corner of the home page or go directly to the Limeade website.
  • Select the Complete Your Health Screening tile on the Limeade home page to access the Physician Fax Form.

You’ll need to download the form online, print it and bring it to your appointment. Simply ask your doctor to fax in the form (as instructed on the cover sheet) to the number listed on the form by March 31, 2015.

Appointments are not required. Be sure to bring your Kohl’s medical plan ID card as well, so the MinuteClinic can file a claim for your visit.

Find a MinuteClinic

Make sure to lookup the closest CVS MinuteClinic because not every CVS store has a MinuteClinic. You can download the CVS mobile app and click the MinuteClinic icon to use the “Find a clinic” feature. Or you can call MinuteClinic customer service at 866-389-ASAP (2727), for specific clinic hours or questions.

Using Lab Results from Your Doctor or Other Retail Convenience Clinic

After your doctor or health care professional submits your form, please allow 72 hours for your results to appear in your account on the Limeade website. Once your results have been uploaded into your wellness account, you will receive a confirmation email informing you that your results are now visible. To view your results, visit the Limeade website. If you do not receive a confirmation email or do not see your results in your wellness account after 72 hours, please contact support@adurolife.com.

On-site or at Kohl’s Wellness Center

You’ll receive your results immediately. A Limeade representative or Wellness Center Nurse will review your risk factors with you to help identify Kohl’s resources and programs that can help you improve or maintain your health.
